Flaky or inconsistant USB connection..

If you find that you can wiggle or push the USB cable upwards and loose connection with the controller a vast majority of the time we find this is due to the male side of the USB connector being on the loose side. What we've found is taking needle nose pliers (or alternatively 'pressing' with a standard screwdriver) to the center of the connector and pinching the connector a bit many times this will solve the issue as the male connector will 'grip' the connector better where it matters most.
